Decision Card- Use this card for each person saved, healed or filled with the Holy Spirit. You can use it to call them to invite to church, pray for them or sending follow up material.

Download this card, copy it, cut it out and give a stack to each member of your witness team. Have the person fill this out when they get saved or healed than hand them into your leader for the follow up team.
